MCB provides online application forms for grants, scholarships, camp, convention, and program services. Most forms are now available online — no paper required.

Grants & Financial Assistance
ACB Convention Grant Application
Apply for an MCB matching grant to attend the American Council of the Blind national convention. Must be an active MCB member.
Adaptive Technology Grant Application
Apply for MCB grants covering up to 50% of qualifying assistive technology purchases. Members up to $3,000 over five years.
Emergency Disaster Relief Fund Application
Apply for emergency gift card assistance if you are blind or visually impaired and have been directly impacted by a declared disaster.
Special Services Application
Apply for MCB's Special Services program covering medical, dental, utility, and other essential social needs.
Health Benefits Program Application
Apply for $45/week income assistance (up to 10 weeks) if you are legally blind and employed in Missouri and have been ill or injured.
Scholarships
Educational Scholarships Application
Apply for MCB's annual educational scholarships for legally blind Missouri residents enrolled in post-secondary education. Up to $2,000 for members.
First Timer Convention Scholarship Application
Apply for scholarship funding covering lodging, transportation, and meals at MCB convention for first-time attendees who are legally blind Missouri residents.

White Cane Program Application
Apply for a free white cane through MCB's White Cane Program for legally blind Missouri residents.
Summer Camp Application Payment
Apply for MCB Summer Camp for MCB members and legally blind Missourians.
Youth Services
Youth Services Grant Application
Apply for MCB Youth Services grants covering adapted educational materials for legally blind children from birth through 12th grade. Up to $225 per year.
Youth Services Grant — Medical Summary
Medical summary form required with Youth Services Grant Application. Must be completed and signed by a licensed physician. Eye report must be dated within 12 months.
Youth Services Event Registration
Register a youth participant (ages 8–18) for MCB Youth Services events. Includes health, vision, and parent/guardian information.
Convention
MCB Convention Registration Form Payment
Register for the annual MCB State Convention. Select meal options, breakfasts, tours, and special accommodations. Pre-registration $10; on-site $20.
Membership
Membership Application Payment
Join MCB as a Member-at-Large ($20/year) or through a local affiliate. Online and paper applications available.
Donations
Angel Wall Donation Form Payment
Donate in honor or memory of someone special. When cumulative donations reach $100, the honoree's name is featured on MCB's Angel Wall tribute.
Staff & Internal
Expense Form for Reimbursement
Reimbursement and expense reporting form for MCB board members and volunteers. Covers mileage ($0.60/mile), meals (max $60/day), and other expenses.
Submitting Supporting Documents
Many program applications require supporting documents (doctor's statements, verification of blindness, receipts, or price quotes). After submitting your online form, please send supporting documents by any of these methods:
